During consultations, I occasionally meet patients with schizophrenia, whose symptoms are usually hallucinations, delusions, or disorganized thoughts. A patient once told the author that he had amnesia and the feeling of “deja vu” at the same time. Because these special symptoms required detailed evaluation, the final diagnosis was changed from schizophrenia to “temporal lobe epilepsy.”

Temporal lobe epilepsy (TLE) is a brain disease that causes the temporal lobe of the brain to emit abnormal brain waves, causing patients to have repeated and involuntary epileptic seizures. Although this is a brain disease, it is closely related to psychiatry because it can cause different psychiatric symptoms. In other words, the symptoms of some patients with schizophrenia or bipolar disorder may be caused by underlying temporal lobe epilepsy.

The clinical manifestations of temporal lobe epilepsy can be divided into 6 types: 1. Sensory (hearing, taste, vision, smell); 2. Emotional (restlessness, fear, mania, depression); 3. Autonomic (abdomen, heart) ); 4. Memory impairment (amnesia, feeling of déjà vu, etc.); 5. Automatism or psychomotor attacks (pharyngeal, oral, simple or complex movements); 6. Confused state of consciousness (insanity, etc.).

The hallucinations experienced by patients with general schizophrenia are usually auditory hallucinations, that is, hearing voices when no one is around, but the hallucinations experienced by patients with temporal lobe epilepsy are usually other senses, such as visual hallucinations (seeing images that do not exist) or hallucinations in patients with temporal lobe epilepsy. The same strange smell is detected in different places. In addition, amnesia, déjà vu, and the feeling of having never seen before (Jamais Vu) are typical symptoms of temporal lobe epilepsy.

Temporal lobe epilepsy may be caused by some damage or scarring in the temporal lobe area of ​​the brain, so EEG examination is the most important auxiliary examination method for diagnosing temporal lobe epilepsy. In terms of medication, patients can choose different anti-epileptic drugs for treatment.
