Reason: Inspectors found 19 violations (seven high priority) including 10 live flies “flying near the back door near the dishwashing area” and the kitchen walk-in cooler, as well as five live cockroaches “on boxes of tahini in the kitchen” and on the shelves outside the cold room. Inspectors also noted, and counted, “50 cockroach droppings on shelves outside the walk-in freezer,” plus four rodent droppings around the same shelf in the kitchen. Finally, inspectors spotted a dead cockroach near the back door of the kitchen and called in an “employee without a hair restraint who was preparing food.” The state discovered more live cockroaches during its second inspection on March 16, keeping the kosher restaurant closed, but following a third inspection later that day, it authorized the restaurant to reopen.
