According to the doctor who answered the question “Which people with cardiovascular diseases are not recommended to fast”, those who have persistent complaints related to cardiovascular diseases such as chest pain and shortness of breath should not fast:

“At the same time, fasting is inconvenient for patients with advanced heart failure, especially those who take high doses of diuretics in the summer. Patients who have had a heart attack in the last six months, chest pain due to heart disease, balloon, stent or bypass surgery in the last six months, uncontrolled or life-threatening arrhythmias, resistant hypertension, i.e. high blood pressure despite drug therapy patients should not fast until their blood pressure normalizes.
