The National Order of Nurses (ONI) deeply regrets “the corporatist and retrograde position of the National Academy of Medicine” in front of the greater improvement in the skills of nurses and in particular IPAs. Communiqué.
“It is with amazement that the National Order of Nurses (ONI) has read the press release from the National Academy of Medicine entitled “ Doing without a medical diagnosis must remain an exception » published on February 15, 2024.
In this press release, the National Academy of Medicine clarifies its position on the advances made possible by the law of May 19, 2023 improving access to care through trust in health professionals. This law, the implementing texts of which have unfortunately still not been published, notably opens direct access to nurses in advanced practice (IPA).
The ONI had widely welcomed this text carried by Madam Deputy Stéphanie Rist, aiming to develop the field of skills of health professionals in order to guarantee local and quality care to all, while it exists in France a proven and lasting shortage of doctors. This already intolerable situation is set to get even worse, due to the aging of the population and the development of chronic diseases, resulting in particular in the increase in the number of people with long-term illnesses (ALD).
It is therefore essential that health professionals mobilize together to provide concrete solutions on the ground. As such, the ONI has always promoted the contribution that nurses might make in the fight once morest the difficulties of access to care in our territory.
The parliamentary debates had also proven that these widely expected developments garnered a strong consensus. This law opened the door to a greater appreciation and increase in the skills of nurses, which will have to be achieved within the framework of the reform of the nursing decree currently under discussion.
Therefore, the ONI can only regret the corporatist and retrograde position taken by the National Academy of Medicine.
RPNs have the skills and knowledge necessary to treat numerous pathologies following having followed demanding dedicated training, which allows them to provide quality care and appropriate treatments to their patients, with autonomy and responsibility, in collaboration with doctors. An IGAS report [1] on the subject as the numerous successful foreign examples of increasing the skills and autonomy of nurses push – contrary to the postulate of the Academy of Medicine – to go further and allow the extension of advanced practice, to the extent of needs of patient care and the aspirations of professionals. This must necessarily involve a regulatory body recognizing the skills acquired in the service of users.
The current situation prohibits us from sticking to such positions, tinged with nostalgia and contempt, which further fracture society, establish a license to treat based on corporatism rather than on skills and formalize care at several speeds. Therefore, let us hope in the future, through interprofessional dialogue, more discernment and a spirit of openness so that we can establish constructive dialogue between health professionals and work together to achieve a more efficient and accessible health system. to all, in responsibility.

We must imagine new solutions and the enhancement of the role of nurses provided for by law is going in the right direction. The IGAS tells us this, like all the examples that are already working abroad. We must move towards greater cooperation between health professionals, with an increase in skills, in the service of patients, this must be our only compass.
