What have they made of you, family and community medicine? You were the gateway to the health system. Their mouths were full in the 80s talking about the specialty… There you are now, neglected for decades, without investments, without objectives, without horizons, with your doctors exposed to the wrath of users and with no one to defend them; with the managers of the autonomous communities unable to restore their lost dignity, with bad contracts and worse working conditions… It is curious that some of these managers come from the same specialty that they do not know how to claim or give prestige to.
Now the health centers are empty, the primary care doctors are fed up with being the girls of the health system. In line with the pandemic, they decided to stand up and stop receiving people in person, taking care of administrative functions in which they wasted their time in a miserable way.

And do you know whose fault it was first? From the Socialist Party when it governed in the 80s, which stripped doctors of authority and filled them with tasks that did not correspond to them; That was when the Ministry of Health still centralized health management and INSALUD offered a minimum level of health equality among Spaniards, before the transfers filled most health services in the autonomous communities with ineffective bureaucrats, with honorable exceptions. There the first stone of a pyramid that has ended up collapsing was removed.
Now, young doctors do not want to go to health centers to train. So that? To charge less than much less qualified personnel, to be disrespected by users, to lack professional horizons and stimuli? A building that has been collapsing for twenty years is not rebuilt in months or years; even less if you lack the experience and knowledge to do so and you work influenced by ideological biases that invalidate almost any initiative.

Return the authority of the teams and the decision-making capacity to primary care physicians. Improve your working conditions and your incentives, your professional career. Inexorably and severely persecute those who disrespect you, those who attack you, those who ignore you. Perhaps in this way they will redirect the specialty and make it attractive again for students and young doctors. As long as this is not done, all the declarations and plans will be a dead letter, as has happened with the last MIR call, in which the increase in the number of positions has not solved anything; in fact, they have remained to cover more than last year.
Pray for the sick, for those who care for them and for family medicine, which we all need.
