The Link Between Subclinical Atherosclerosis and Cardiovascular Risk: Key Findings from Xuanwu Hospital Study

[Voice of Hope September 1, 2023]Recently, a study conducted by scholars from Xuanwu Hospital Affiliated to Capital Medical University in China showed that subclinical atherosclerosis was detected in more than half of the patients with medium and low cardiovascular risk.

In other words, this group of people has a higher cardiovascular risk than traditional risk stratification.

Brachial-ankle pulse wave velocity (baPWV) and brachial artery blood flow were measured in 421 participants without a history of atherosclerotic cardiovascular disease (ASCVD) between October 2016 and January 2020. Mediated vasodilation (BFMD), the mean age of the patients was 57.05 years old, and 58.9% were male. And all participants were divided into low, medium and high risk groups according to the Framingham and China-PAR risk models.

The study showed that among low-, intermediate-, and high-risk patients stratified by the Framingham and China-PAR risk models:

The proportions of abnormal baPWV were 42.9%, 70.1%, 85.7% and 40.4%, 71.4%, 89.7% respectively;

The proportions of abnormal BFMD were 43.8%, 68.5%, 77.3% and 44.9%, 72.1%, 76.6%, respectively.

In multivariate analysis, age and blood pressure were independent predictors of subclinical atherosclerosis in low- and middle-risk groups.

Combining ASCVD risk models with subclinical atherosclerosis parameters may predict cardiovascular risk more accurately and guide preventive measures than ASCVD risk models alone, the researchers said.

Previously, the latest analysis of the China Acute Myocardial Infarction Registry (CAMI) study led by Yang Yuejin of Fuwai Hospital found that more than half of the patients with acute myocardial infarction in China had a low-medium risk of atherosclerotic cardiovascular disease (ASCVD) score at the time of onset. crowd. Among patients <55 years of age with acute MI, nearly half (47%) were low risk.
