The Occitadys network, a territorial network of caregivers in the Occitanie region, specializes in the diagnosis and monitoring of language and learning disorders, including attention deficit disorder with or without hyperactivity (ADHD). Its model makes it possible to shorten access times to care, while moving away from the fee-for-service pricing method. Funded thanks to a legal provision introduced in 2018, article 51 of the Social Security financing law, allowing the testing of new health organizations, it is about to be generalized, after three years of experimentation. Meeting with its president, the Toulouse pediatrician Thiébaut-Noël Willig.

First of all, a word on the recommendations of the High Authority of Health regarding the management of ADHD in children, to which you contributed?

They fill a void and are fundamental when we know that ADHD is the most common disorder in the field of neurodevelopment. French child psychiatry has long been psychoanalytically inspired, partly preventing the recognition of ADHD, as was the case for autism. Some practitioners still use it, even if it is decreasing in favor of a neurocognitive approach. In any case, it is an enforceable document which helps guide the practice and organization of care.

Why did you launch Occitadys?

Twenty years ago, neurodevelopmental disorders [TND] were not taught in medicine, even though comorbidities reach 80%. So how can we make a diagnosis or coordinate care when the needs are real? For my part, I have trained myself, but it is far from being a generality: according to the health professional consulted, it is the lottery for families.

Thanks to article 51, we were able to set up and finance a training structure recognized by the National Agency for Continuing Professional Development [ANDPC]. Between 2021 and 2023, we trained 250 primary care general practitioners and pediatricians to diagnose simple TND, or to identify complex ADHD requiring referral to secondary care doctors. The objective is to do the same in the next two years, to reach 10% of doctors trained in the region. In addition, we have, in Occitanie, 65 doctors specializing in TND, including ADHD, which is huge. We also provide training in parental guidance as well as the initiation of medication treatments for ADHD. A network is the assurance of sharing common values, and that starts by speaking the same language.
