Each day that passes shows us a little more that France is facing a growing and desperate gap between the needs and the supply of care. The 20 million French people who consult emergency departments each year, according to the department of research, studies, evaluation and statistics, are testimony to this.
The causes are multiple: aging of the population, increased incidence of diseases linked to our lifestyles, medical progress extending life expectancy, and the consequences of a Malthusian policy of restricting the training of doctors during decades.
What to do today? Preparing for the future is essential, but we must forget the idea that training more doctors is an appropriate response, because this will only have an effect in more than ten years: this amounts to giving up on solving the problem during this period, unbearable renunciation for patients. We must accept that innovative local initiatives, often effective, sometimes – due to a lack of global thinking – only move the problem from one territory to another.
Create medical time
We must also renounce, for ethical reasons, plundering other countries, often in greater difficulty, which does not prevent us from taking a sympathetic look at the situation of doctors holding a foreign diploma who are already practicing in our country. territory.

Ultimately, there are only three areas of action left. The first consists of ensuring that young doctors, recently graduated from our faculties and whose number has never been so large, actually practice what they were trained for. Indeed, some favor exercises far from their specialty, and less priority, such as aesthetic medicine, or even leave medical practice. It is a subject of attractiveness rather than coercion, if only because coercion is not effective in times of scarcity.
The second is to create medical time with the doctors we have available. Let’s forget, here once more, the idea of making them work more, we just need to look at their current working time to understand that it is illusory and that the situation can only get worse, with the younger generations no longer accepting – and it is legitimate – the conditions of exercise of their elders. Let’s not wait for telemedicine, digital health and artificial intelligence to permanently resolve the situation, because they also consume medical time.
