Rocky Mountain Spotted Fever (RMSF) Outbreak in Tecate, Mexico: CDC Health Advisory

The Centers for Disease Control and Prevention (CDC) is issuing a health advisory to notify health care providers and the public of an outbreak of Rocky Mountain spotted fever (RMSF) in the United States among individuals with recent travel or residence in the town of Tecatein the state of Baja California, in Mexico. As of December 8, 2023, five patients have been diagnosed with confirmed PMRF since the end of July 2023; all had traveled to or resided in Tecate in the two weeks prior to illness onset. PMRF is a serious, rapidly progressive and often fatal disease transmitted by the bite of infected ticks, although many patients do not remember being bitten by a tick. Doxycycline is the treatment of choice for patients of all ages.

FMSR is endemic in many of Mexico’s northern border states, including but not limited to Baja California, Sonora, Chihuahua, Coahuila, and Nuevo León. Health care providers should consider FMSR in their differential diagnosis in patients who have reported recent travel to Tecate, Mexico, or other areas of northern Mexico and subsequently develop signs or symptoms of an unexplained severe febrile illness. It is recommended to consider initiating doxycycline on epidemiological and clinical suspicion, without waiting for the results of a confirmatory laboratory test. Early treatment with doxycycline saves lives.
