“Dr. Pei Pei, do you think dry eye syndrome will occur after cataract surgery?” Dry eye syndrome is an ocular surface disease that affects the tear film, leading to tear film instability, eye discomfort, and visual impairment. Some patients often suffer from intermittent blurred vision and mistakenly believe that cataract surgery can solve the problem. They do not know that dry eye syndrome already exists along with cataracts. If you ignore dry eye symptoms and undergo cataract surgery, you may increase the chance of dry eyes.
Daily experience shows that approximately one-quarter to one-third of patients experience dry eye syndrome after cataract surgery. During cataract surgery, two microincisions need to be made to remove the cataract, but these incisions will inevitably cut through the corneal nerve. The function of these nerves is to sense whether there are enough tears on the surface of the eye. When there is a lack of tears, the corneal nerves tell the brain to produce more tears. When nerves are severed, the feedback loop between the cornea and the brain is temporarily interrupted, reducing tear production and disrupting the homeostasis of the tear film and ocular surface. However, this feedback modulates once the corneal nerves heal. Therefore, pure cataract surgery usually only temporarily affects the ocular surface or causes dry eyes. Eye drops used before and after surgery contain preservatives that may irritate the ocular surface. Exposure to irrigation, drying, and the light of the operating microscope during surgery may cause some damage to the corneal and conjunctival surfaces and aggravate the symptoms of ocular surface disease. Even if ocular surface disease is mild before surgery, worsening symptoms may persist for months after surgery, negatively impacting vision.
Antibiotic drops are given to patients after surgery to prevent infection and inflammation from occurring. If you experience dry eye discomfort after taking the medication, use preservative-free artificial tears to lubricate the ocular surface for at least 10 to 15 minutes after using the antibiotic drops. Dusty, windy and dry conditions should be avoided after surgery as central heating systems heat the air and evaporate the tear fluid. When using your computer, make sure you blink frequently.
