Outbreak at Tough Mudder Event in Sonoma, California: Symptoms, Causes, and Treatment

According to US health sources, regarding 300 people who participated in a recent Tough Mudder event, a mud obstacle course held in Sonomain Californiafell ill and presented with rashes, various lesions, fever or flu-like symptoms.

These cases might be due to several infectious agents, including staphylococci, but the main culprit is the bacteria AeromonasMore precisely A. hydrophila (multiple wound cultures revealed the presence of this bacterium which was sensitive to several antibiotics, including ciprofloxacin, doxycycline and trimethoprim-sulfamethoxazole).

The Tough Mudder event was held at Sonoma Raceway on August 19-20, 2023, and symptoms of the infection appeared within 12 to 48 hours. The race resulted in major exposure of the skin to the mud. The event featured 21 obstacles on one of the race days, including a mud-soaked course under barbed wire, rope climbs on a muddy slope, a knee-deep pool of mud to cross, and an obstacle called “pit” which, according to one participant, smelled of manure.

A. hydrophila is a bacterium that lives in aquatic environments and soil. It can infect humans, animals, birds, fish and marine reptiles. In man, Aeromonas has often been considered a cause of gastroenteritis. But this bacteria can cause diarrhea, soft tissue infections, necrotizing fasciitis and sepsis.
