Half of people with skin diseases suffer from sleep disorders. In addition, certain dermatoses are particularly difficult to live with, such as Verneuil’s disease, also known as hidradenitis suppurativa.
Nearly half, precisely 42.3%, of patients with skin diseases suffer from sleep disorders, according to a first study on this subject, announced by the European Academy of Dermatology and Venereology at its 2023 congress. (EADV, Berlin, Germany/October 2023).
Researchers from PROJECT ALL, an international research initiative, conducted a survey of 50,000 adults in 20 countries (including 4,000 in France) to assess the impact of skin diseases, such as atopic dermatitis/eczema, psoriasis , acne, vitiligo, rosacea, hidradenitis suppurativa, etc. The main symptoms that affected sleep in patients with skin diseases were itching (60%) and burning or stinging sensations (17%). In addition, patients more frequently felt fatigue upon awakening (81%, compared to 64% in the general population), periods of drowsiness during the day (83%, compared to 71%), and tingling sensations in the eyes (58%, versus 42%) and repeated yawning (72%, versus 58%).
The main author of the study, Dr Bruno Halioua, dermatologist (Paris), adds: “In the future, caregivers should be encouraged to include questions regarding possible sleep disorders when interviewing patients with skin conditions. This study is the first to highlight the importance of early detection and effective management of sleep-related problems in this population. »
An impact beyond sleep
PROJECT ALL also found that these sleep disorders had broader implications on the quality of life of patients with skin disease. Indeed, almost half (49%) of them reported reduced productivity at work, compared to just one in five participants (19%) without a skin condition.
Skin pain is also cited by 25% of people with acne, 15% of those with rosacea, 16% with psoriasis, 15% with hidradenitis suppurativa, 15% with eczema and 11% with hand eczema. and 11% vitiligo.
Focus on Verneuil disease
The study looked more specifically at the daily impact of hidradenitis suppurativa, also known as Verneuil’s disease, named following the surgeon who first described it. It is a skin condition causing skin abscesses, painful deep nodules and scarring. It affects around 1 in 100 people. The average diagnosis time remains too long, on average 8 years, according to the French Society of Dermatology. This disease manifests itself in hair follicles where the sweat glands that produce sweat are located, such as the groin area, buttocks, breasts and armpits. Its exact cause remains poorly understood. It is often difficult to manage, although symptoms may improve or even disappear with treatment.
Study results reveal that 77% of hidradenitis suppurativa patients experienced stigma due to their illness, while 58% experienced ostracism or rejection from others . More than half of patients reported that people avoided them physically (57%) and socially (54%) because of their condition. These experiences had significant consequences for patients, influencing their perceptions of themselves, their relationships, and their daily lives.
« The stigma associated with this disease has a profound impact on patients’ lives and can perpetuate a vicious cycle of isolation and non-compliance with treatment. By raising awareness, we can collectively work to create a more inclusive society, thereby improving treatment adherence and reducing the burden on patients », concludes Dr. Halioua.
