For who ?
Prevention assessments are intended for all people at key ages of life, that is to say between:
- 18 and 25 years old;
- 45 and 50 years old;
- 60 and 65 years old;
- 70 and 75 years old.
When ?
For all age groups concerned, prevention assessments will be accessible early 2024 among doctors, midwives, nurses and pharmacists.
If you are between 45 and 50 years old and you live in the Hauts-de-France region, you can participate in the pilot program for this system from October 2023 with volunteer health professionals or via health centers. health examination.
How much does it cost ?
Prevention assessments at key ages of life are 100% covered by Health Insurance for all social security holders, without advance costs.
1. I prepare my prevention report by answering a self-questionnaire
Quick and easy to complete, the self-questionnaire will allow me to take an initial look at my lifestyle habits and my health priorities. At the beginning of January, it will be possible to complete this self-questionnaire on My Health Space.
To note : if I cannot complete it alone, I can do it during my appointment.
2. I make an appointment with the healthcare professional of my choice
Preventive health assessments can be carried out by doctors (and in particular your GP), nurses, pharmacists and midwives.
As part of the deployment of the pilot program, I inquire with the healthcare professional of my choice whether they carry out prevention assessments.
To note : if you already know the health professional, you can make an appointment according to the usual procedures, specifying that it is a prevention assessment, because the duration of the assessment is longer than a traditional consultation.
3. I benefit from personalized advice to adopt, at my own pace, lifestyle habits that are more favorable to my health
For 30 to 45 minutes, you discuss your lifestyle habits with the healthcare professional in order to determine your health priorities together. The prevention assessments will be an opportunity to address your needs and desires, for example in terms of diet, physical activity, sleep, etc. You will be able to discuss the subjects that are most important to you, such as your mental health. , your sexual health, questions related to menopause or loss of autonomy, cancer prevention and screening…
My prevention assessment allows, thanks to the advice of a health professional, to have the keys to identify and modify your behaviors, so that they are more favorable to your health. During this discussion, you will define, with the help of the health professional, one or two of your health priorities on which you wish to act.
4. I complete my Personalized Prevention Plan with the healthcare professional and, if follow-up is needed, they direct me to the appropriate professionals or structures
At the end of the assessment, you write, with the healthcare professional, your personalized prevention plan, indicating your priority health objectives and the concrete actions adapted to your daily life to implement at your own pace. This personalized prevention plan will then be sent to your attending physician (if he is not the healthcare professional carrying out your assessment).
In the event of an untreated or unknown illness, you are referred directly to a doctor (preferably your GP).
