It started with a red spot on the nose and the initially reassuring diagnosis from the family doctor: sunburn. However, when the nose of the 40-year-old German turned black and started to rot, he was taken to the hospital and received a frightening multiple diagnosis.
A PCR test confirmed monkeypox infection, which the patient also showed as large boils around his mouth and the rest of his body.
But what was the cause of the black, putrid nose?
Tests for sexually transmitted diseases finally provided certainty and a slap in the patient’s face. Result: A longer lasting syphilis and an advanced HIV infection.
As a result, the patient’s immune system, who had never been screened for STDs before, was significantly weakened. Necrosis, where tissue begins to die off, had an easy time on the 40-year-old’s nose.

The German’s severe case of monkeypox was reported in the medical journal “ Infection“ documented. It states: “Most cases of monkeypox infection have been reported as mild and controlled HIV infection does not appear to be a risk factor for severe cases. However, this case illustrates the potential severity of monkeypox infection in the context of severe immunosuppression and untreated HIV infection.”
The doctors, led by Dr. Christoph Boesecke from the University Hospital in Bonn gave the patient Tecovirimat – a drug used to treat monkeypox – for a week.
This was enough to treat the body’s wounds. The nose improved, the swelling reduced. The report did not provide any further information about the man’s recovery.
