To UNITED STATES, following 10 years of vaccination coverage (VL) close to 95% nationally, the VL of the measles, mumps and rubella (MMR) vaccine; vaccine once morest diphtheria, tetanus and acellular pertussis (DTPa); vaccine once morest poliovirus (polio); and varicella vaccine (VAR) decreased by approximately 1% during the 2020-21 school year and fell an additional percentage point during the 2021-22 school year, to establish at approximately 93%.
Review Morbidity and Mortality Weekly Report has just published an article which takes stock of the CVs of kindergarten students and the vaccination exemptions concerning them in 49 states and the District of Columbia (DC) during the 2022-2023 school year.
1. Vaccination coverage
At the national level, vaccination coverage according to vaccines was as follows:
CV of MMR 2 doses: 93.1% (extremes of 81.3% [Idaho] at ≥98.4% [Mississippi]), with a CV ≥95% reported by 13 States and <90% by 12 States and DC; CV of DTPa: 92.7% (extremes of 81.0% [Idaho] at ≥98.4% [Mississippi]), with a CV ≥95% reported by 11 states and <90% by 14 states and DC; polio CV: 93.1% (range 81.8% [Idaho] at ≥98.4% [Mississippi]), with CV ≥95% reported by 13 States and <90% by 12 States and DC.CV of VAR: 92.9% (extremes of 80.7% [Idaho] at ≥98.4% [Mississippi]), with CV ≥95% reported by 11 states and <90% by 12 states.
In the 2022-23 school year, the national VL among kindergartners remained approximately 93% for MMR, DTPa, polio, and VAR, similar to that in the 2021-22 school year . This national result masks the continued decline in VL seen in most states for all vaccines compared to the 2021-22 school year.
National MMR coverage among kindergarten students remained below the 95% target for the third consecutive year. A national MMR coverage of 93.1% in the 2022-23 school year translates to approximately 250,000 kindergartners who are at risk for measles.
2. Vaccination exemptions
The number of kindergarten children benefiting from vaccination exemptions for one or more vaccines increased by 0.4% compared to the 2021-2022 school year to reach 3% (extremes: < 0.1% in Virginia- Western and 12.1% in Idaho). This is the highest exemption rate ever recorded in the United States.
Non-medical exemptions which represent more than 90% of the reasons for exemption and alone explain the increase in the national exemption rate.
Exemptions increased in 40 states and D.C. and increased by at least 1% in seven states.
Concerning RORof the 36 states and DC with a CV < 95% in the 2022-2023 school year, 10 states reported that more than 5% of children were exempt. This means that for these 10 states it is impossible to achieve an MMR CV ≥95% even if all non-exempt kindergartners in 2022-23 were vaccinated, compared to four states in 2021-22.
Overall, 2.5% of students were enrolled in kindergarten subject to vaccination updates (provisional registration or grace period) in the 28 states reporting this data (0.5% [Géorgie et Hawaï] at 9.2% [Arkansas]).
3. Conclusions
The population of students affected corresponds to those who were eligible for these vaccines for the first time at the height of the pandemic. Nationally, vaccination coverage of kindergarten students remains below pre-pandemic levels and exemptions have increased. The authors conclude that the covid pandemic has had an impact on routine childhood vaccination, whether due to a real increase in hesitancy or an increase in non-medical exemptions due to barriers to vaccination or for convenience. .
