As the New England Journal of Medicine (NEJM) reports, the 64-year-old initially suffered from periodontitis, which is why his doctor prescribed the antibiotic “Clindamycin”.
Around 20 days later, the patient from the USA returned to see his doctor. He complained of a greenish discoloration of his tongue that had been there for two weeks.
The man then underwent a course of fluconazole, an antifungal drug used to treat fungal infections. They wanted to be sure that the 64-year-old was not suffering from oral thrush. But the remedy didn’t work.

Shortly therefollowing, a doctor noticed that long, threadlike papillae had formed on the man’s tongue. These caused the tongue to appear hairy.
However, the 64-year-old had no other symptoms such as taste disorders or pain.

Combination of smoking and antibiotics was apparently to blame

The diagnosis: The smoker suffered from a “lingua villosa”, the so-called hairy tongue.
According to that NEJM Hairy tongues are a benign disease of the muscular organ covered by a specialized mucous membrane.
Affected patients are often smokers. Other risk factors include dehydration, poor oral hygiene or, as in the case of the US patient, the use of antimicrobials. The hairy tongue can turn black, brown, yellow or green in patients.

The 64-year-old was advised to gently scrub his tongue with a toothbrush four times a day. He was also advised to stop smoking.
Although the patient did not accept the last recommendation, the condition of the tongue normalized once more following six months.
