The Institute of Public Health of the Republic of North Macedonia reported a confirmed case of Crimean-Congo hemorrhagic fever (FHCC) in a 27-year-old woman from the village of Kuchica, municipality of Acceptance.
The patient presented the first symptoms of the disease (high temperature up to 38.6°C, severe headache, malaise and vomiting) on July 21. She had been bitten by a tick two days earlier. She was hospitalized in the Infectious Diseases Department of the KB Shtip from July 23 to 25. Due to the deterioration of her condition, the patient was transferred and hospitalized at KIBFS in Skopje. The disease progressed rapidly and on the evening of July 27 the patient died, hours following Crimean-Congo hemorrhagic fever was confirmed.
A search for contact cases was made in the family environment and in all the health establishments where the patient stayed. All contacts have been screened and an appropriate disease risk classification has been established. Of a total of 67 identified contacts, five people were assessed as high risk, 24 medium risk and 38 low risk. In accordance with WHO guidance, blood was drawn for testing from four of the contacts identified as high risk; a negative result for CCHF was obtained for all four cases.
The first outbreak of this disease described in the Republic of North Macedonia was a family outbreak in 1971, in a family from the village of Chiflik – Tetovo, during which 13 people – family members – fell ill, and the disease ended with the death of two of them (mortality was 15.3%). After this epidemic, a case was confirmed in the laboratory in 2010. Since there are other natural foci in Macedonia where the disease circulates in nature among ticks, wild and domestic animals and birds, this disease represents a potential danger because it is possible for a person to become infected by the bite of an infected tick.
Among neighboring countries, the disease is endemic in Kosovo and sporadic cases have been recorded in several other European countries.
