Collaborative Diabetes Screening and Support Services in the Haute-Yamaska Region

HEALTH. Diabetes screening and support for people with diabetes will no longer be done in silos in the region. The organization Les Diabétiques de la Haute-Yamaska, the Cégep de Granby and the CIUSSS de l’Estrie-CHUS are now working together by offering a continuum of services to individuals affected by this silent illness. A new collaboration which begins with the upcoming three days of screening.

For a person who claims to be diabetic, obtaining a result is a real obstacle course these days. To avoid endless delays for these people waiting for confirmation, the organization Les Diabétiques de la Haute-Yamaska ​​tried to approach the Cégep de Granby and its nursing department. An approach which ultimately resulted in a partnership between the two organizations which also included collaboration with health professionals from the CLSC Yvan-Duquette responsible for the Agir sur santé program.

Concretely, nursing students will offer three half-days of screening (capillary blood glucose and blood pressure reading) at the Cégep de Granby Health Clinic on October 25, November 8 and November 25. In the event of a positive test (high blood sugar), the person whose life is in danger will be sent to the emergency room. If the situation is not alarming, she will be directed to the 811 line so that she can speak to a doctor and will be enrolled in the Agir sur sa santé program (promotion of healthy lifestyle habits for people aged 18 and over with a chronic illness: diabetes, obesity, high blood pressure, etc.).

For the Cégep de Granby and Les Diabétiques de la Haute-Yamaska, the formula proposed to the population is advantageous for both organizations. In addition to the on-the-job training component for nursing students, the Cégep also contributes to somewhat relieving congestion in the health network with this service offering. “This partnership allows us to help a part of the population who does not have a family doctor, who is aware that they may have diabetes and who does not have the means to be screened,” said indicated Véronique Jacques, nurse in charge of the health clinic at Cégep de Granby.

For its part, Diabétiques de la Haute-Yamaska ​​will play its role as a reference organization: presentation of the services offered, activities and others.

“For us, it is important to do screening precisely to avoid complications. This led me to try to see where I might find nurses and I thought regarding nursing at Cégep de Granby. And one thing led to another, everything lined up. We are all complementary and that can help the system which is currently clogged. For the population, it’s helpful,” declared Sandy Lapointe, development advisor at Les Diabétiques de la Haute-Yamaska.
