CH Le Vinatier: a cooperation protocol for monitoring patients on Clozapine

The Le Vinatier Hospital Center, a reference establishment in psychiatry, has just implemented the local cooperation protocol “Follow-up nursing consultation in a Medical-Psychological Center for patients on Clozapine”. This innovative initiative, inspired by the HPST (Hospital, Patients, Health, Territories) and ASAP (Acceleration and Simplification of Public Action) laws, represents an important milestone in the care of schizophrenic patients.

A cooperation protocol involves the transfer of activities or acts of care from one healthcare professional to another to enable coordinated collaboration between healthcare professionals by redistributing their responsibilities to provide high-quality care to patients. The main objective is clear:
– Reduce the time limit access to care,
– Improve the quality and the security care,
– Strengthen the SKILLS nurses,
– Increase their professional attractiveness,
–    Et optimize medical time for patients requiring complex care.

The Le Vinatier Hospital Center has integrated the deployment of cooperation protocols at the heart of its future 2024-2028 establishment project, under the theme “Promoting new professions and new professional practices”. The main objective is to reduce delays in accessing care, improve the quality and safety of care, strengthen the skills of nurses, increase their attractiveness and retain these professionals while optimizing medical time for patients requiring complex care.

In this context, the cooperation protocol “Follow-up nursing consultation in CMP of patients on Clozapine” has been set up. The nurses there monitor stabilized chronic schizophrenic patients treated with Clozapine alternating with the referring doctor, carrying out two consultations out of three. Clozapine is an antipsychotic reserved for the treatment of resistant schizophrenia, and its use follows strict recommendations which very precisely regulate the prescription and, therefore, make it optimally safe.

The delegation of acts includes in particular the verification of the conformity of the Blood Count (NFS) with the continuation of treatment, the validation of the monitoring log, the renewal of the prescription for Clozapine at the same dosage and the prescription of examinations additional services such as blood tests and electrocardiograms. Nurses who have completed theoretical and practical training, as well as medical validation of their acquired knowledge, and who have five years of experience in psychiatry, including two in CMP, are authorized to exercise these functions.

On Wednesday October 4, 2023, a nurse from the Le Vinatier Hospital Center carried out the first consultation in this format. The experience was positive for both the nurse and the patient. She also expresses her satisfaction with the support and confidence of the institution in this innovative project, while expressing the hope that the dynamic of increasing the skills of nurses will continue thanks to this system.

The Le Vinatier Hospital Center and its teams are proud of this major innovation which is also an important historical step in the practice of psychiatric care. They undertake to continue the deployment of cooperation protocols within their establishment, considering their use in other care services in order to continue to improve the quality of care offered to their patients.
