“Analysis and Reflection on Patient-Centered Health Care in Mexico: Insights from Seminar on Medicine and Health”

Medical care and personal health services are fundamental to the well-being of society; For this reason, the Seminar on Medicine and Health of the Faculty of Medicine of the UNAM, in collaboration with the National Academy of Medicine of Mexico, the Mexican Academy of Surgery and the National Regulatory Committee of General Medicine, dedicated the fourth edition of the Seminar “Analysis and Reflection on the Health System in Mexico» to this topic.

On April 28, in the auditorium «Dr. Alberto Guevara Rojas» and through Facebook Liveand YouTube, Dr. Rubén Argüero Sánchez, moderator of the event and Professor Emeritus of the Faculty of Medicine, recalled that the objective of this Seminar is to bring together academics and health professionals to disseminate the knowledge that is generated as a result of it.

On his occasion, Dr. Enrique Wolpert Barraza, former General Director of the ABC Medical Center, referred to patient-centered medical care as that collaboration between all health professionals, the patient and their family to guarantee that medical decisions respect the needs and preferences of the patient and support him to participate in his care.

He then presented the key elements he considers to achieve person-centered care: having empathy, understanding individual preferences, discussing personal and medical goals for shared decision-making, accompanied by active listening and thoughtful conversation that responds to the questions from the patient and his family.

To exemplify patient-centered care, he shared the measures that were implemented at the ABC Medical Center during the care of patients with COVID-19. To respond to the lack of contact between family members, they facilitated communication through mobile devices; to make staff feel safe, they ensured the availability of protective equipment and made available a 24/7 mental health program; In addition, they used technology to give remote consultations; academic activities were carried out online and learning through simulators was strengthened.

Dr. José de Jesús Arriaga Dávila, General Director of Orange Box, Health Innovation Consulting, agreed that it is necessary to reform the health system, since he considered that the traditional hospital model, which centralizes resources and knowledge, is little accessible, expensive and therefore not feasible for the future.

To rethink the system, he proposed using the triple goal model, which includes population health, which seeks to reach communities and organizations trying to generate well-being and prevention; the second section implies the reduction of the per capita cost, segmenting the population according to their needs and risk factors and making the processes more efficient to avoid wasting resources; Finally, it would be necessary to improve the care experience, increasing safety, effectiveness, opportunity for access, and efficiency, without neglecting equity and always centered on the patient.

Also, to provide options that favor innovation in health systems, he shared some enablers focused on population health, business intelligence, the focus on processes through the Lean Healthcare and technology management.

Commenting on the presentations, Dr. Marco Antonio Navarrete Prida, Coordinator of High Specialty Hospitals of the Ministry of Health of the State of Mexico, mentioned that to reform the health system into one centered on people, three aspects are required: first, continuity , from diagnosis to palliative care; second, personalized prevention, according to individual risk factors using information and artificial intelligence; Finally, personal health services must be exhaustive, that is, reach the maximum care possible.

He also warned that “to improve the quality of life and increase life expectancy we must start investing in health.”

For his part, Dr. Enrique Ruelas Barajas, President of the International Institute of Health Futures, stressed that during personal service care it is essential to provide medical care as a team, not just in a group.

In addition, he proposed defining human care, which encompasses people-centered care, as a professional, honest, friendly, trustworthy, and respectful relationship that builds trust and considers others as whole persons. And he raised the need to pay attention to students, who must learn to provide humane and quality care.

Finally, Dr. Abraham Campos Romero, Director of Innovation and Research at Salud Digna Para Todos, representing Juan Carlos Ordóñez Zamudio, General Director of Salud Digna Para Todos, shared his comments on the presentations, acknowledging that areas of opportunity will always be found. in the health system, and highlighted the role of non-governmental organizations such as Salud Digna, which support not only by reducing service times and with affordable prices, but also by collaborating in research and academia.

The next session of the Seminar will take place at the Palace of the School of Medicine on June 2 and will address the topics “Unification of Health Systems” and “The Health System for the well-being of Mexico.”
