A laterality error in radiotherapy caused a radiation protection incident at the University Hospital of Tours. A patient received a treatment on the breast which did not include a lesion.
The Radiotherapy Center of the Bretonneau Hospital, of the CHU of Tours, alerted the Nuclear Safety Authority (ASN) on June 26, 2023 regarding a significant radiation protection event which occurred within this establishment. This incident was caused by a laterality error which resulted in the contouring of a breast which should not be treated because it contained no lesion. The left breast of a patient was therefore the subject of a treatment plan of 28 sessions each delivering 2.25 Gy.
It seems, according to the ASN report, that this error is linked to erroneous information contained in the report of the initial medical consultation. The patient, who was made aware of the error and its potential consequences, was offered a new treatment plan for her diseased breast. Given the overdose of the region wrongly treated and the potential risk of side effects, the ASN classifies this event as level 2 on the ASN-SFRO scale of events in radiotherapy, graduated from 0 to 7 in increasing order of seriousness. .
