Innovative DiVa Project: Intensive Monitoring of Stroke and MI Patients at Beaune Hospital

The Beaune hospital center is actively involved in the DiVa experiment (Dijon Vascular Project), an innovative project aimed at ensuring intensive monitoring of patients who have suffered a stroke or myocardial infarction (MI). Professor Maurice Giroud, one of the leaders of this experiment, highlights the significant progress in Côte-d’Or in the treatment of these pathologies.

The Beaune hospital center is actively participating in the DiVa experiment (Dijon Vascular Project), an ambitious project aimed at ensuring joint intensive monitoring of patients who have suffered a stroke or myocardial infarction (MI). This project brings together health professionals, notably nurses, doctors and pharmacists, both hospital and private, in the regions of Burgundy and Grand-Est, including Dijon, Beaune, Semur-en-Auxois, Châtillon, Langres and Chaumont.

A new organizational model for intensive monitoring
As part of DiVa, a new organizational model is being tested in order to optimize available medical time. This intensive monitoring of post-stroke and post-MI patients, lasting two years, is carried out under the close supervision of cardiologists and neurologists, in collaboration with the treating physician. Specifically trained nurses provide this monitoring according to a protocol common to both conditions, supplemented by pharmaceutical interviews. This approach aims to detect signs of recurrence or complications early, thus allowing rapid adaptation of treatment.

Challenges and advances in Côte-d’Or
Professor Giroud, one of the representatives of the DiVa experiment, highlights the significant progress in Côte-d’Or in the treatment of strokes. “ In Côte-d’Or, the treatment of the first stroke benefits from a high level of quality, with a significant reduction in mortality of 40% over the last 20 years. However, further efforts are needed to improve the post-stroke phase. Re-hospitalizations remain too frequent due to a lack of adequate monitoring following the acute phase of the disease: the health system is under pressure, with a shortage of doctors, knowing that it takes 12 years to train a professional. health in this area. » In this context, the DiVa experiment focuses on the training of hospital and private nurses, who monitor patients over two years with alternating consultations every three months. Pharmacists also contribute to this approach by adapting treatments, which makes it possible to strengthen the early detection of complications.

Towards a national generalization
This experiment carried out within the Territorial Hospital Group (GHT) “21-52” aims to evaluate its impact both in urban areas, such as Dijon and Beaune, and in rural areas, notably Semur-en-Auxois, Châtillon- sur-Seine, Chaumont and Langres. Its main objective is to reduce recurrences and disabling complications for patients, with the aim of reducing the rate of unplanned re-hospitalizations in emergency departments.

Encouraging results and fruitful collaboration
This interregional collaboration gave positive results on the 895 patients followed, with active participation from the two hospital sites, the Dijon University Hospital and the Beaune hospital center. The trend is encouraging: the number of recurrences is decreasing thanks to intensive monitoring provided by hospital nurses (IDEH) and private nurses (IDEL) trained in the prevention and early detection of complications and recurrences. In addition, the pharmaceutical interviews carried out by hospital or community clinical pharmacists aimed to strengthen therapeutic adherence and reduce iatrogenic complications.
If the results continue to confirm the effectiveness of this model, it might be extended to the entire French territory from 2025, with the possibility of including the monitoring of other pathologies.
Through these innovative initiatives and this fruitful collaboration between the Dijon University Hospital and the Beaune hospital center, the Clément-Drevon Foundation plays an essential role in promoting medical innovation and excellence in care in Côte-d’ Gold.
