Long Covid: anxiety or depressive disorders to watch out for

In prolonged forms of covid-19, the involvement of the nervous system is important, points out the Academy of Medicine. In a position paper, the Sages formulate 4 recommendations, including the systematic search for a depressive and/or anxiety disorder.

The available data show that the SARS-CoV-2 virus has neuroinvasive potential, that its neurotropism is relatively limited but that it can be neurovirulent in certain patients. The damage to the nervous system associated with Covid-19 in the acute phase manifests itself in the form of encephalopathies, strokes and possible damage to the peripheral nervous system.

The involvement of the nervous system is important in prolonged forms. Now recognized by health authorities, “long Covid” is identified as a common condition complicating the course of SARS-CoV-2 infection. Its clinical expression, polymorphous and sometimes confusing, raises questions regarding its mechanism. The clinical modes of expression suggest a broad participation of the nervous system beyond an almost omnipresent cognitive complaint. On this level, we can remember:
– Discretion or even the absence of neuropsychological deficit in the populations studied;
– The preeminence of attentional disorders, the most frequent among the deficit items when they exist;
– The concomitance of anxious and/or depressive elements in a significant proportion.

In this context, the reality of patients suffering from acute neurological damage and patients suffering from long COVID
invite :
– a coordinated research effort to identify the different mechanisms involved,
– screening for cognitive deficit in the face of a cognitive complaint,
– systematically looking for a depressive and/or anxiety disorder,
– specialized multidisciplinary care in which the attending physician must play a key role.
