The operating room is fictional. Twenty-two errors were distilled there. “The goal of the exercise is to reinforce the work we do on a daily basis, to strengthen our vigilance“, immediately explains Sébastien Villon, head of the operating room at the South Health Center in Le Mans. Every year, 30,000 interventions are carried out in this private clinic.
Vigilance in “daily work”
Errors ranging from poor labeling of products, poorly positioned needles or syringes to anomalies on the mannequin. “His intubation is not fixed. He was supposed to have surgery on his right foot, he has a bandage on his left foot“, note Marion and Maéva, two nurses in the recovery room. “The patient’s identity is incorrect, his patient is missing a letter, it’s a small detail but it’s important“, notes Maéva.
All the staff of this private clinic were invited this Friday to participate in this workshop. “This exercise allows us to question ourselves. We also do things on a daily basis, often a little quickly, we can lose track so it’s good to remember all that“, confirms the nurse.

“The idea is that from administrative staff to surgeons, everyone can participate in this workshop, so that everyone understands what may be an error in someone else’s area of expertise. Recovery room nurses have spotted certain errors because they are in their field but not others, the idea is that everyone can have a panel of possible errors“, notes Sébastien Villion.
Especially since errors are not allowed in this health establishment where 170 patients are treated per day. “Our lack of vigilance can have an impact on the patient, more or less serious. With this exercise, we resume the vigilance that we have in the care of our patients, identity, pharmacy, medication errors, be careful“, concludes the head of the operating room.
