Gonorrhea, syphilis, chlamydia: what STIs (sexually transmitted infections) can cause sore throat?
Oral sex (fellatio, cunnilingus) can be a vector of several sexually transmitted infections (STIs), which in some cases can affect the throat or mouth.
La chlamydia (chlamydiosis of the throat) and gonocoque (gonorrhea) are, for example, likely to settle in the throat, but they are mostly not painful. “In rare cases, however, gonococcus will cause symptoms similar to those of angina,” indicates Dr. Lelièvre. Gonorrhea and chlamydia are the two most common sexually transmitted diseases in France, and their prevalence has continued to increase in recent years. Syphilis, on the other hand, is painless, but causes large lymph nodes in the neck and sometimes lesions in the back of the throat.
Genital herpes can also be transmitted during fellatio, and cause painful lesions in the partner’s mouth.
Finally, infectious mononucleosis (known as kissing disease) is transmitted through saliva, therefore potentially through oral sex, and causes significant sore throats. “But most often, mononucleosis is transmitted by kissing and not by fellatio,” adds the gynecologist.
The majority of STIs that can be caught through genital oral sex are asymptomatic or nearly asymptomatic, meaning they have no symptoms, and therefore no sore throat.
“But sore throats in the days following fellatio may also not have no connection with sexual practiceand simply be a symptom of viral angina for example,” specifies Dr. Lelièvre.

Throat pain following vigorous fellatio can also be of mechanical origin and no longer infectious. Small bruises can appear at the level of the soft palate, linked to strong suctioncausing a barometric depression.
These are micro-hemorrhages from the small blood vessels in the palate, which may be completely painless or slightly tender. It’s completely benign and usually goes away within a week. Dr. Florence Lelièvre, gynecologist
Sore throat: how do I know if I have a sexually transmitted infection?
In the presence of infectious symptoms (sore throat, burning sensation or others) following unprotected oral sex, it is important to carry out a screening for’prevent the infection from getting worse and can lead to following-effects or complications that are more difficult to treat later.
The screening methods then depend on the STIs sought. They can be :
- a blood test,
- a clinical examination of the genitals with possibly a local sample (urethral in men and vaginal for women),
- a urine analysis,
- a pharyngeal swab, at the level of the throat.
These different examinations can be carried out by the attending physician, a gynecologist, a midwife or a medical biologist in a laboratory.
Treatment: how to treat a sexually transmitted infection?
Treatments for gonorrhea and chlamydia are relatively simple and are done by means ofantibiotics which will be prescribed by the doctor following the results of the examinations and/or samples. It is very important to also treat sexual partner(s)since both diseases can have significant consequences on their health if left untreated. For example, chlamydia is the leading cause of infertility in women..
The treatment of the syphilis also relies on antibiotic therapy, most often benzathine penicillin G “delay”, more or less prolonged depending on the time since the patient has been infected.
