Difficulty swallowing, irregular stools, gastric reflux, chronic bloating, can indicate a dysfunction of motility, these muscular movements essential to the digestive system. Update with the Motility Center of Bumrungrad International Hospital

What are motility disorders?

A motility disorder refers to an impairment or dysfunction of normal muscle movements in the digestive system. These muscle movements, called “motility”, are essential for the proper functioning of the gastrointestinal system, allowing the coordinated movement of food and liquids through the different parts of the digestive tract, from the mouth to the anus.

These disorders, involving different organs from the esophagus to the rectum, include:

Difficulty swallowing Irregular stools, including chronic constipation and faecal incontinence. Gastroesophageal Reflux Disease (GERD) – Symptoms: constant burning/tight throat, chronic cough or chest pain. Chronic Abdominal Bloating – Mainly caused by gastrointestinal dysfunction.

The diagnosis of these diseases requires great expertise, through the performance of specialized examinations by highly qualified doctors. Bumrungrad Hospital offers services covering a range of gastrointestinal issues, as follows:

Disorders of the esophagus (swallowing, chronic cough, non-cardiac chest pain or difficulty swallowing) Disorders of the stomach (indigestion, too fast/too slow gastric emptying, chronic vomiting) Disorders of the colon (chronic constipation, irritable bowel syndrome ) Disorders of the small intestine (intestinal pseudo-obstruction, poor intestinal motility, bacterial overgrowth in the small intestine) Disorders of the rectum and dysfunction of the pelvic floor (faecal incontinence, lack of control of the muscles of the pelvic floor, delayed colonic transit)

Medical technology and equipment

The Gastrointestinal Motility Center uses the following modern medical technologies:

1. Manometry:

A medical technique that measures pressures in different parts of the digestive system to assess muscle coordination and motility to aid in the diagnosis of motility disorders.

There are 4 main types of manometry:

> Esophagus: Measures muscle contractions in the esophagus to diagnose problems like reflux or achalasia.
> Antroduodenal: Evaluates motility of the stomach and duodenum, helping to detect gastric emptying disorders.
> Anorectale : Analyzes anal and rectal muscle pressures to diagnose incontinence or constipation.
> Colic: Measures colon contractions to identify issues like irritable bowel syndrome.

2. Esophageal pH-impedance test – 2 methods:

A small catheter with a pH sensor is placed into the esophagus through the nose and then left in place for 24 hours to digitally measure and analyze the amount of acid and non-acid reflux. Capsule endoscopy involves swallowing a small electronic capsule. This capsule sends out radiofrequency signals as it passes through the digestive system, picked up by external receptors on the patient’s body. Once the data has been collected, the capsule is evacuated naturally. Non-invasive, no surgery or traditional endoscope.

3. Hydrogen breath test (H2 test)

Detects common digestive issues like lactose intolerance, excessive growth of bacteria in the small intestine (SIBO), fermentation of undigested sugars, malabsorption of glucose and lactulose. This test measures hydrogen in the breath following ingesting a sugar solution, providing useful diagnostic information in a non-invasive way.

4. Saliva flow test

Determines if there is too little saliva, possible causing chronic pain or burning in the throat, leading to symptoms similar to acid reflux.

5. Anorectal rehabilitation by biofeedback

Intended for people suffering from constipation and faecal incontinence.

Constipation: About 40% of cases are due to poor defecation technique with overexertion and insufficient contraction of the anal sphincter muscles. Doctors will guide patients to adopt the right method. Fecal incontinence: Common in the elderly, it can cause a lack of perception during defecation. Doctors will help patients contract the anal sphincter muscles properly through modern techniques.

Bumrungrad Hospital emphasizes accurate diagnosis and early detection before serious symptoms occur, to increase the chances of successful treatment and full recovery.
