Seoul St. Mary’s Hospital, sharing cases of adverse drug reactions in the hospital
A woman in her 60s diagnosed with a rare disease called POEMS syndrome needs attention as she experienced another side effect with the drug administered for the treatment of uncontrolled fluid overload.
The Pharmaceutical Department of Seoul St. Mary’s Hospital recently shared a case of adverse drug reaction in the hospital.
First of all, POEMS syndrome is polyneuropathy ((P)olyneuropathy), organ hypertrophy ((O)rganomegaly), endocrinopathy ((E)ndocrinoapthy), monoclonal gammopathy ((M)onoclonal gammopathy) or M-protein (M It is a rare disease in which (S)kin defects appear.
Looking at the shared adverse events, a 62-year-old female patient diagnosed with POEMS syndrome was admitted to Seoul St.
The patient underwent autologous hematopoietic stem cell transplantation following pre-transplantation chemotherapy with the multiple myeloma drug ‘Melparan’, and antibiotics were started because neutropenic fever accompanied by shock occurred while managing the lowest blood count.
Subsequently, following administering the antibiotic ‘Maxfim Inj’, the administration was changed to ‘Tazoferan Inj’, and acute renal damage accompanied by metabolic acidosis occurred, so the dose was reduced along with the implementation of continuous renal replacement therapy. Afterwards, administration was started by adding the antibacterial drug ‘Teicosin Inj’, and following discontinuing the long-term renal replacement therapy, he changed to hemodialysis and administered a reduced amount of Tazoferan inj.
Afterwards, a rash accompanied by itchiness and fever occurred all over the body, and there was a possibility of a rash caused by teicosine, so administration was discontinued, systemic steroid injections were started, and DRESS syndrome was diagnosed.
In this regard, the Ministry of Pharmacy said, “Since the patient’s drug administration caused severe DRESS syndrome, systemic steroids were administered, so this was judged to be another medically important situation and evaluated as symptomatic.” The symptom is reflected in the permission, but the effect of the concomitant drug Tazoferan Injection cannot be ruled out, so it was evaluated as possible (Possible).”
On the other hand, in the case of Teicosine, it is stated that DRESS syndrome, exfoliative dermatitis, toxic epidermal necrolysis, erythema multiforme, and mucocutaneous ocular syndrome may appear in the domestic approval conditions.
DRESS related to skin and subcutaneous tissue has been reported with Tazoferan.
