On the second day following menstruation, the level of prostaglandin increases and the contraction and expansion of the uterus become active.

[사진=게티이미지뱅크]“The whole body is throbbing, the lower body seems to come out from the waist down,,, the pain in the lower abdomen as if someone squeezed and pressed the organs with their hands… .It’s two days of pain. Why does it hurt the most the day following menstruation starts?”

Many women complain of excruciating pain on the second day of menstruation. Why is the pain peaking on the second day?

It has to do with hormones, especially prostaglandins. Prostaglandin is a powerful physiologically active hormone that has functions such as constriction and dilation of blood vessels, promotion or reduction of platelet aggregation, detection of pain signals in spinal nerves, and induction of labor.

According to Dr. Fatima Dauphoy, an obstetrician and gynecologist interviewed by the British daily newspaper Thesun, women’s hormone levels are highest during the first few days of menstruation. In particular, it reaches its peak on the second day, and at this time, as progesterone decreases, menstrual cramps become the worst and the pain becomes the most severe. As bleeding continues and the endometrium is shed, prostaglandin concentrations decrease, which relieves symptoms.

As the endometrium breaks down, prostaglandins, hormone-like chemicals, are released from the body, which stimulate the uterus to contract and help expel menstrual blood. Prostaglandins constrict blood vessels in the uterus, reducing the amount of oxygen that uterine tissue receives, which causes cramps and pain.

There are other factors that are perfectly normal and affect the level of pain on the second day of menstruation. Low estrogen levels and heavy bleeding are one reason.

“The second day is usually associated with greater menstrual blood flow,” said British obstetrician Dr Ashfaq Khan. It can cause discomfort.”

Depending on the cause of menstrual cramps, it is divided into primary and secondary

Dysmenorrhea (dysmenorrhea) is divided into primary and secondary depending on the cause. Primary dysmenorrhea is a symptom that is not a major problem because there is no gynecological disease, and secondary dysmenorrhea is a symptom caused by other diseases.

As explained above, primary dysmenorrhea is caused by prostaglandin, which causes the most pain on the second day. It is caused by the build-up of substances called prostaglandins in the endometrium during menstruation. Increased production of prostaglandins causes the muscles of the uterus to contract, causing pain. If you take a drug that inhibits the production of prostaglandin for 2 to 3 days, dysmenorrhea is alleviated.

Secondary dysmenorrhea is pain caused by uterine fibroids or endometriosis. Primary dysmenorrhea usually appears within 1 to 2 years of menarche. Secondary dysmenorrhea, on the other hand, does not occur until several years following menarche.

Professor Park So-yeon of the Department of Obstetrics and Gynecology at Ewha Womans University Seoul Hospital said, “If you have dysmenorrhea following the age of 20 or if the pain has recently worsened, if the pain appears before menstruation starts and continues to hurt following menstruation ends, it is highly likely that you have secondary dysmenorrhea.” Even if there is pain, it can be secondary dysmenorrhea with a gynecological disease, so gynecological care is needed.”
