fatalizer (One-Handed Sword) – Unique Affix: Lucky Strike: You have up to X% chance to inflict X Shadow damage to nearby enemies and reduce their damage done by 20% for 5 sec. Grand father (Two Handed Sword) – Unique Affix: Increases your critical hit damage by 60-100%. For the other characteristics of this weapon, higher values can be rolled. Face of Andariel (Helmet) – Unique Affix: Lucky Strike: Up to 15-20% chance to trigger a poison nova that deals X poison damage to enemies in the area of effect for 5 sec. harlequin crown (Helmet) – Unique Affix: You gain 10-20% damage reduction. You also gain +4 ranks for all skills. Melted Blessed Heart (Amulet) – Unique Affix: Your maximum resources are increased by +30%. When you take damage, 3-8 resources are removed each time instead of 1% of your HP. Ring of the Starless Skies (Ring) – Unique Affix: Each Basic Skill cast consecutively reduces the resource cost of your next Basic Skill by 8-12%, up to a maximum of 40%.
What also characterizes these super rare unique items is that they are not only very rare, but they all drop to 820 area power (the current maximum, except for a few items at 840), making them the most powerful items in the game. Plus, they all have boosted stats which, for the most part, pair well with affixes.

They drop from level 85 (and above) enemies and generally where unique items can drop – so they’re NOT limited to specific bosses or locations. According to Blizzard, every time a unique item drops, there’s a “very very” low chance that it’s one of the 6 rarest items.
