Four medical centers have filed applications with the North Carolina Department of Health and Human Services (NCDHHS) to develop a Medicare-certified home health care agency in response to a determination in the State Facility Plan Medical 2023 for a health care agency in Forsyth County.
The applicants and their proposals are:
Personal Home Care of North Carolina LLC, proposing to develop a Medicare-certified home health agency in Winston-Salem, NC: PHC Home Health-Forsyth. The project is expected to cost $65,000 and would be completed in April 2024.
Well Care Home Health of Forsyth Inc. is proposing to develop a Medicare-certified home health care agency in Kernersville, North Carolina: Well Care Home Health of Forsyth County. The project is expected to cost $100,000 and would be completed in July 2024.
Forsyth Memorial Hospital Inc. and Novant Health Inc., are proposing to develop a Medicare-certified home health agency in Clemmons, North Carolina: Novant Health Home Care-Forsyth. The project is expected to cost $231,000 and will be completed in July 2024.
Five Points Healthcare of NC LLC is proposing to develop a Medicare-certified home health agency in Winston-Salem, NC: Aveanna Home Health-Forsyth. The project is expected to cost $115,000 and will be completed in January 2024.
A public hearing for these projects will be held June 16 at 9 am in the auditorium of the Reynolda Manor Branch of the Forsyth County Public Library, 2839 Fairlawn Drive NW in Winston-Salem.
