After the Mixed Joint Commission on the Rist law aimed at improving the health system through trust and simplification, the National Order of Nurses denounces the inconsistency of the choice not to retain the Professional Territorial Health Communities (CPTS) in the law. The ONI expresses its incomprehension in the face of this decision which establishes, according to him, ” access to care at two levels in the territories and deals a major blow to the deployment of CPTS, by a lack of confidence of parliamentarians in health professionals« . The Commission has certainly adopted provisions which constitute real progress : the evolution of the skills of all nurses in France in the field of wounds and healing; their integration into ongoing care; the opening of direct access and First Prescription to Advanced Practice Nurses within the framework of a coordinated exercise.
But the choice made not to reintegrate the Territorial Professional Health Communities into the law (CPTS), which are precisely the structures that would make it possible to sustain the coordinated exercise of professionals serving patients, is both incoherent and incomprehensible, since this provision had been passed unanimously by the Assembly before be rejected by the Senate. This position was also supported by patient associations and all of the scientific work on CPTS showed its benefits. Corporatism, insults, threats and disinformation will have got the better of the patient’s interest.
This decision considerably weakens the balance and scope of the text. It springs from a corporatist reflex which gives priority to the defense of the interests of some rather than the best interests of patients, and in particular of the 6 million French people who do not have access to a attending physician. This is really medicine at two speeds: between those who have a doctor and those who do not! As a reminder, the first mission of the CPTS is to facilitate access to a general practitioner and this deletion, made without any argument, goes once morest this objective.
The extremely limited experimentation retained in the law also testifies to a lack of consideration for the experiments carried out by professionals in the field. who, on a daily basis, collaborate effectively in the CPTS in the service of patients, and who hoped to be able to take a new step with this law.
This de facto creates a 2-speed France for access to care between the areas where the experiments are taking place and those which are maintained in the status quo.
Finally, this choice of non-reintegration of CPTS testifies to a lack of recognition of the skills and commitment of nurses who are always asked for more – in terms of working hours, personal involvement, acceptance of the arduous nature of their job – without giving them the elements of consideration to which they are entitled.

Because the non-choices of today bring the health system of our country into the wall. As a result, the National Order of Nurses is calling for public authorities to take a leap forward to finally take the necessary decisions to increase the skills of nurses in the service of patients, in particular through the urgent reform of the nursing skills decree, but also by the end of the protocol linking IPA and doctors, a protocol which has no legal basis and which undermines the free choice of patients. Neither professionals nor patients can wait any longer. By wanting too much to protect the corporate interests of another time, we establish de facto a two-speed care and we create forgotten territories of the Republic in terms of access to care. ».
